Community Care PackagesThe Community Aged Care Packages project is funded by the Commonwealth Department of Health and Ageing. Community Aged Care Packages are planned and co-ordinated packages of community care services to help people with complex care needs remain living in their own home. They are designed for each individual and are based on their particular needs. The types of services which may be offered in a Community Care Package are detailed in Section 12.5 of the Community Care Subsidy Principles 1997 and may include: • Personal Care To be eligible for Community Aged Care Places a person must be assessed by the Age Care Assessment Team (ACAT) as being eligible for Hostel level of care but who is wishing to remain living at home. These people would otherwise be eligible for residential care (low). There is funding for 35 Community Aged Care Packages for the Macleay. Contact: Anne - Program Administrator |
