Community Options

Community Options is a client focused service aiming to provide flexible responses to a client’s need. This service provides case management and high level support to clients eligible for the HACC Program. Clients are most often referred by other service providers or ACAT. Community Options is based on a brokerage model of service and services are delivered to meet individual and changing client needs. Assistance can be managed by liaising with other community care services and supplying extra services to meet client needs.

Community OptionsCase Management refers to assistance received by a client from a specific worker who is formally responsible for managing the planning, coordination, monitoring and reviewing of the delivery of HACC services and support across a range of agencies for clients with complex needs.

A client contribution towards the cost of service provided is discussed and determined at the time of assessment. A client will not be refused service because of inability to pay.
