ComPacks

ComPacks is a package which supports clients within their own homes, while recovering from hospital. The package is available for up to 6 weeks following discharge from hospital. Your ongoing needs will be assessed by the Community Options case manager.

Who is eligible?

People who are currently in hospital and who are assessed as needing two or more community services to support them at home once discharged.

Referrals are made by hospital staff, which include:-

  • Social workers
  • Occupational Therapists
  • Discharge Planners
  • Nurses
  • Doctors

What is provided?

Case management:

A Community Options case manager is allocated to you on referral to the ComPacks program.

They will be your contact throughout the program and assist you to regain your independence.

The case manager can asist you to access services such as:

Domestic Assistance

  • House cleaning
  • Washing and ironing
  • Assistance with pets
  • Assistance with shopping
  • Transport to and from apopintments

Personal Care

Assistance with bathing/dressing and grooming

Meals

Assistance with meal preparation or linkage to with Meals on Wheels.

How much will a ComPacks package cost?

You will have to make a contribution and this cost will depend on your current income.

What happens when the ComPack finishes?

Before the ComPack finishes, the case manager will where possible:

  • Link you with ongoining services if required.
  • Organise placement on waiting lists if necessary
  • Discuss private service options if required.
